According to one of the comments on youtube it was sung by Jim Kweskin of the Jim Kweskin Jug Band fame. His greatest hits on amazon includes ‘Ukulele Lady’ (unfortunately played on the Guitar!! )

My pc died recently , so the chord sheet was done on Google Docs which, to be fair, seems rather good, and has the advantage of being able to save files straight to PDF - nice)
